When Finasteride Stops Working: Understanding the Plateau and How to Push Past It

For many men, the first year on finasteride feels like turning a corner. Shedding slows. Photographs start to look more flattering. Then, somewhere around the 18-month or two-year mark, the momentum stalls. The hair isn't falling out at the same alarming rate, but it isn't thickening further either. Progress has plateaued—and the frustration that follows is one of the most common concerns raised in dermatology and hair loss consultations across the United States.
This experience is not a sign that finasteride has failed. It is, in most cases, a predictable biological event with identifiable causes. Understanding why it happens is the first step toward knowing what, if anything, can be done about it.
What Is a Treatment Plateau, Clinically Speaking?
A finasteride plateau refers to a period during which a patient is maintaining consistent medication adherence but is no longer experiencing measurable improvements in hair density, coverage, or overall scalp health. It is important to distinguish this from treatment failure. Hair loss that has been halted is still a meaningful clinical outcome—the alternative, without treatment, would likely be continued and progressive thinning.
That said, a true plateau can also represent a point at which the drug's mechanism has reached its ceiling within a given patient's biological context. Finasteride works by inhibiting the enzyme 5-alpha reductase, which converts testosterone into dihydrotestosterone (DHT). Elevated DHT is the primary driver of androgenetic alopecia—commonly known as male pattern baldness. By reducing DHT levels, finasteride slows or stops follicle miniaturization. However, it does not permanently repair follicles that have already undergone significant damage.
The Role of Genetic Sensitivity
One of the most significant factors determining whether a man plateaus—and how quickly—is his individual genetic sensitivity to androgens. The androgen receptors in hair follicles vary considerably from person to person. Some men's follicles respond robustly to DHT suppression, showing clear regrowth even in areas that had thinned substantially. Others have follicles with such heightened androgen sensitivity that even markedly reduced DHT levels are sufficient to continue the miniaturization process, albeit more slowly.
This genetic variability helps explain why two men on identical finasteride regimens for the same duration can have dramatically different outcomes. For those with high follicular androgen sensitivity, the plateau may arrive earlier and be more pronounced. Clinical research has identified specific polymorphisms in the androgen receptor gene that correlate with treatment responsiveness, though genetic testing of this kind is not yet standard clinical practice in most US dermatology offices.
Hair Cycle Timing and What It Means for Long-Term Outcomes
The human hair growth cycle—comprising the anagen (growth), catagen (transition), and telogen (resting) phases—also plays a role in how plateaus develop. When finasteride first begins reducing DHT, dormant follicles that had been miniaturized may re-enter the anagen phase, producing the initial regrowth that many patients notice in months six through eighteen.
Over time, however, those follicles cycle through their phases repeatedly. Follicles that are genetically programmed to eventually stop producing terminal hair will do so regardless of DHT suppression—particularly if they have already undergone several years of androgen-related damage prior to treatment. The hair cycle, in essence, has a finite number of productive rotations for compromised follicles, and finasteride can extend but not indefinitely preserve that window.
DHT Rebound and Incomplete Suppression
Another clinical factor worth examining is the consistency and degree of DHT suppression achieved by finasteride at standard dosing. The standard 1 mg daily dose reduces serum DHT by approximately 60 to 70 percent in most patients. For some men, this level of suppression is sufficient to halt progression and promote regrowth. For others, it may not adequately protect the most sensitive follicular zones—particularly the crown and vertex, which tend to be the last areas to respond and the first to stall.
There is also the question of what happens when adherence is imperfect. Even occasional missed doses can allow DHT to rebound temporarily, which may be enough to accelerate miniaturization in highly sensitive follicles. Patients who experience a plateau should honestly evaluate whether their adherence has been truly consistent before attributing stagnation to a pharmacological ceiling.
Evidence-Based Strategies for Breaking Through
If a genuine plateau has been confirmed—through consistent adherence and photographic documentation over at least three to six months—several evidence-based strategies may be worth discussing with a healthcare provider.
Combination Therapy with Minoxidil
The most well-supported adjunct to finasteride is topical minoxidil. The two drugs operate through distinct mechanisms: finasteride addresses the hormonal driver of hair loss, while minoxidil acts as a vasodilator that prolongs the anagen phase and stimulates follicular activity. Clinical studies have consistently shown that the combination produces superior outcomes compared to either agent alone. For men who have been on finasteride monotherapy and hit a wall, adding minoxidil—whether the 5% topical solution or foam—is typically the first evidence-based recommendation.
Reassessing Dosage
In some clinical settings, providers have explored whether a higher finasteride dose might achieve greater DHT suppression. The 5 mg dose, originally approved for benign prostatic hyperplasia, suppresses DHT more completely than the 1 mg formulation. While this approach carries a different side effect consideration profile and is not universally endorsed for hair loss specifically, it has been employed in certain cases where standard dosing appears insufficient. This is a conversation best had directly with a dermatologist or urologist.
Lifestyle Optimization
Emerging evidence suggests that several lifestyle factors may influence the efficacy of hair loss treatment. Chronic psychological stress elevates cortisol, which can disrupt the hair cycle and potentially counteract some of the benefits of DHT suppression. Sleep deprivation, nutritional deficiencies—particularly in iron, zinc, and biotin—and poor scalp circulation have all been associated with suboptimal hair health outcomes. While these factors alone do not cause androgenetic alopecia, they may compound the limitations of pharmacological therapy.
Low-Level Laser Therapy
FDA-cleared low-level laser therapy (LLLT) devices have demonstrated modest but measurable efficacy as an adjunct in several clinical trials. The mechanism is not fully understood, but photobiomodulation appears to stimulate follicular metabolism and may extend the anagen phase. For men on finasteride who have plateaued, LLLT represents a low-risk addition to their regimen.
Managing Expectations Realistically
Perhaps the most important clinical insight about plateaus is also the most difficult to accept: for many men, maintaining current hair density without further loss is the realistic long-term goal of finasteride therapy. The drug's primary strength lies in preservation, not restoration. If a plateau has arrested what would otherwise have been ongoing thinning, the treatment is doing exactly what the evidence supports it to do.
For men who find this insufficient—particularly those with significant baseline loss—hair transplantation, performed after medical therapy has stabilized the donor area, may be worth exploring as a complementary intervention.
The plateau is not the end of the road. It is a signal to reassess, consult with a qualified provider, and consider whether the current treatment protocol remains optimally configured for where you are in your hair loss journey.
